The Missouri Arthritis and Osteoporosis Program has developed a plan of action for the sustainability of Self-management Education Programs (SEP). Known as the Roots Project, the goal is to provide a foundation for improving the health and well-being of individuals and families, especially those living with chronic conditions. There are 10 key areas of strategy and planning. They include: revenue, marketing, referral network, competition, service operation, evaluation, organizational support, community support, advocacy, and resource linkages.
