The Missouri Arthritis and Osteoporosis Program is currently in the process of creating a sustainability plan, the Roots Project, to take a look at chronic disease self-management programs and how to sustain them. Throughout the plan, we would like to include direct quotes from individuals involved with the Chronic Disease Self-Management Program (CDSMP) and our other evidence-based programs. In order to help facilitate this feedback, we have created an Evidence-Based Program Success Story form which you can easily fill out and submit to our office.
Success stories are just that, stories of personal success achieved as a result of participating with one of our programs. This could be direct or indirect health changes, improved community programming, or any number of other impacts that the programs you have participated with have made on you! You can either click on "Success Stories" under the Roots Projects Documents on the left side of our sustainability page (www.rootsproject.net), or you can click here to be taken directly to the form.
We would like to continue to utilize these success stories for other projects as well. If we choose to use pieces of your success story, we will contact you in advance.
