New paperwork for use with all group MAOP interventions (CDSMP, Tomando Control de su Salud, Walk With Ease, and Arthritis Foundation Exercise Program) can be found in the left column of the Leader Resources page. This paperwork includes a new Leader Training Roster (under header "Master Trainers – Leader Training Forms") and a new Participant Attendance Sheet (aka course roster) (under header "Leader Forms for Participant Courses:).
Also, if you are new to MAOP paperwork, don't forget about the Course Participation Questions (Yellow Reach Card), also in the left column of the Leader Resources page. A participant attendance sheet must be filled out for each course series of all MAOP interventions, and each participant must fill out a Yellow Reach Card at the completion of each course series for all MAOP interventions.
