Living a Healthy Life (CDSMP) and DSMP Leaders and Trainers: PLEASE NOTE THE NEW CHANGE in paperwork as of 12/10/13. We are NO LONGER using the Consent Form. We will retain the use of the group leader script (modified to drop any reference to a Medicare study). The group leader script will continue to inform participants that their data is being shared with ACL/NCOA and we will continue the privacy procedures to protect the data. All paperwork remains the same EXCEPT for the consent form.
