The Growth Action Plan worksheet within Missouri’s Sustainability Toolkit has been updated based on feedback from users over the past year. The Toolkit, published on March 31, 2012, remains the same in content. The formatting of the Growth Action Plan Worksheet appendix was modified to be more user-friendly. In addition to the electronic version of Missouri’s Sustainability Toolkit, you can now download the Growth Action Plan worksheet in electronic and print formats in the righthand sidebar.
Missouri Sustainability Toolkit
The Missouri Arthritis and Osteoporosis Program has developed a plan of action for the sustainability of self-management Education Programs (SEP). Known as the Roots Project, the goal is to provide a foundation for improving the health and well-being of individuals and families, especially those living with chronic conditions. There are 10 key areas of strategy and planning. They include revenue, marketing, referral network, competition, service operation, evaluation, organizational support, community support, advocacy, and resource linkages.
