- AF Participant Release Form (1 form per NEW participant)
This form can be used for all AF programs and events.

- Participant Information Survey Card (by Region) (1 form per participant)
Paperwork Submission: Please submit all forms to your RAC Coordinator at the completion of each course. Paperwork should be submitted in full, completed, and in a timely fashion.
