One form of evaluation must be completed for each participant in order for them to count for reach. Suggestions of how to track this are below:
- WWE-Individual Pocket Journal
- AF WWE Individual Participant Evaluation Form
- WWE-Individual Phone Evaluation
Each participant must also submit a survey for each class they take.
- Participant Information Survey Card (by Region) (1 form per participant)
Paperwork Submission: Please submit all forms to your RAC Coordinator at the completion of each course. Paperwork should be submitted in full, completed, and in a timely fashion.
